  • those aged 18 years to under 65 years in clinical risk groups as defined by the Green Book, chapter 19 (chronic respiratory disease, chronic heart disease and vascular disease, chronic kidney disease, chronic liver disease, chronic neurological disease, diabetes and adrenal insufficiency, immunosuppression, asplenia or splenic dysfunction, morbid obesity BMI 40+)
  • pregnant women

We hope that you will support us by having your vaccination at the surgery.

EMBRACING THE NHS APP

The Charlton Team would like to encourage and assist you as our patient to download and embrace the NHS App. The NHS App has evolved significantly and there are so many things you can now do on the NHS App, reducing the need for you to ring the surgery. We know that our phone lines are extremely busy and this is frustrating for you as a patient as well as the team who are struggling to answer the huge volume of calls in a timely manner whilst still offering good patient care. We would welcome patients using the App to access information and services, freeing up the phone lines for those without NHS App access to be able to contact us if they need to.

If you would be willing to work with us to give this a try please download the NHS App and follow the instructions. You will be prompted to input a LINKAGE KEY. We can provide this to you as your GP practice. Please email charlton.reception@nhs.net requesting a linkage key for the NHS App and one of the team will respond as soon as possible. Alternatively, you can telephone the surgery to request the linkage key information. Our team will ask some security questions and then email you with the linkage key. If you would prefer to visit us at the surgery to obtain the linkage key, please visit the surgery after 10am with Identification and a member of our team will assist you. Thank you for working with us to move forward in improving the ways you can access services at Charlton.

CHARLTON IS WORKING CLOSELY WITH LOCAL COMMUNITY PHARMACISTS

Pharmacies across Shropshire, Telford and Wrekin are now offering a range of extended services to help people gain access to medical advice and treatment at a place more convenient to them. The new Community Pharmacy Extended Care Service allows people to visit a local pharmacy instead of their GP practice for a range of minor illnesses. A list of participating pharmacies can be HERE
