NEW PATIENT REGISTRATION
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill/driving license.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Medical treatment is available from the date of registration. Please remember to include your NHS number on the patient registration form. This can be obtained from your previous GP or by clicking HERE
Please note that you are registered with the practice, not with a specific doctor. However, you are free to consult with any available doctor of your choice.
When you register with the practice you will also be required to complete a Medical Questionnaire for New Patients. This is because it can take a considerable amount of time for us to receive your medical records. This can be accessed by clicking the link below to download and print the form. Once completed please return with your completed and signed registration form along with your photographic ID (if available) and proof of address in person to the surgery. Please note that if you are unable to provide identification documents, this does not prevent you from registering with the surgery.
